Washington Dental Service "Lose The Right Paper" Sweepstakes

1. NO PURCHASE NECESSARY. A PURCHASE OR TRANSACTION WILL NOT INCREASE YOUR CHANCE OF WINNING.
   
2. ADMINISTRATION; SPONSOR. This sweepstakes is being administered and sponsored by Washington Dental Service (WDS), 9706 4th Avenue NE, Seattle, WA 98115-2157.
   
3. ELIGIBILITY. Open only to legal residents who are at least 18 years of age and are currently residing in the following states: Alaska, California, Georgia, Idaho, Illinois, Missouri, Oklahoma, Oregon, Pennsylvania, Texas, Virginia and Washington. Void in Florida, New York, Rhode Island and where prohibited by law. Employees and independent contractors of Washington Dental Service and its affiliates, as well as their immediate family members (spouse, parent, child, sibling and their respective spouses), regardless of where they reside, and those living in the same household of each, whether or not related, are not eligible. Additionally, WDS reserves the right to disqualify any person from participation in the sweepstakes if WDS is notified by a WDS group client that participation in the sweepstakes by an employee of such organization would conflict with company policy, or where such participation would, in the sole opinion of WDS, compromise or give the appearance of compromising the integrity of the sweepstakes.
   
4. PROMOTIONAL PERIOD. Promotion period is from 12:00:00 a.m. PST November 7, 2009, through 11:59:59 p.m. PST May 6, 2010. WDS will select the winner in a single, random drawing from the eligible entries received on or around May 14, 2010.
   
5. HOW TO ENTER. There are two ways to enter: (1) Registered users of DeltaDentalWA.com: (WDS Brokers, WDS Employers, WDS Subscribers, and WDS Dentists) that sign into DeltaDentalWA.com and choose to activate paperless statement options will automatically receive one entry into the drawing OR (2) If you are not a registered user of DeltaDentalWA.com, or if you do not wish to receive paperless statements from WDS, complete the entry form located at www.DeltaDentalWA.com/PaperlessForm and mail it to WDS, Attention: Lose the Right Paper Sweepstakes, 9706 4th Avenue NE, Seattle, WA 98115-2157. Mail in entries must be postmarked by May 6, 2010 and must be received by May 13, 2010. All entries become the exclusive property of WDS and will not be returned or acknowledged. WDS and its affiliates, officers, directors, agents and representatives will not be responsible for difficulties associated with confirmation of paperless statement options because of telephone, electronic, hardware, software, network, Internet or any other computer or other communications related malfunctions or failures. WDS is not responsible for incomplete, lost, illegible, late, postage-due or misdirected entries.
   
6. PRIZE. One prize of $10,000 (to be awarded in the form of a check). Prize is not transferable and non-exchangeable. Odds of winning depend on the number of eligible entries received.
   
7. DRAWING. Prize will be awarded in a random drawing on or about May 14, 2010 from among all eligible entries received.
   
8. GENERAL CONDITIONS. Potential winner will be notified via e-mail and/or telephone. All decisions by WDS regarding the prize, this sweepstakes and the winner are final, binding and conclusive on all matters. WDS and any other partner or affiliate shall not be liable for late, lost, misdirected or unsuccessful efforts to notify the winner. Potential winner will be required to return an Affidavit of Eligibility, Release of Liability and, where not prohibited by law, a Publicity Release within seven (7) days of attempted delivery of same. Non-compliance within this time period or return of any prize notification as undeliverable will result in disqualification without further notice and an alternate winner will be chosen. Federal, state and local taxes are the sole responsibility of the prize winner. Winner does not need to be present to win. No purchase or payment of any consideration is necessary to enter the sweepstakes. Only one entry per person is allowed. Multiple entries by any one person will result in a disqualification. By entering the drawing, each entrant agrees to comply with these Official Rules. Noncompliance with any entry procedure will result in disqualification. Decisions of WDS as to the interpretation of the Official Rules shall be final and binding in all respects. WDS reserves the right to cancel, suspend and/or modify the drawing if any fraud, technical failures or any other factor beyond WDS’s reasonable control impairs the integrity or proper functioning of the drawing, as determined by WDS in its sole discretion. In such event, WDS reserves the right to award the prize at random from among the eligible entries received up to the time of the drawing. WDS reserves the right in its sole discretion to disqualify any individual it finds to be tampering with the entry process or the operation of the drawing or to be acting in violation of these Official Rules or in an unsportsmanlike or disruptive manner. WDS’s failure to enforce any term of these Official Rules shall not constitute a waiver of that provision. The laws of the state of Washington will govern all disputes regarding this sweepstakes.
   
9. CONSENT TO PUBLICITY; RELEASE OF LIABILITY. By accepting the prize, the winner consents to the use of his or her name and likeness for publicity purposes without further compensation. Winner will be featured on DeltaDentalWA.com and in other public-facing communications. By entering, all entrants release and hold harmless Washington Dental Service and its partners, employees, agents, officers, directors, representatives and affiliated companies from all liability or any loss, damage or claim of any kind with respect to, or in any way arising from, this sweepstakes.
