The following items are needed to initiate coverage through Washington Dental Service:

• Master application. A completed master application should include a signature by an authorized representative of the group.
• Enrollment forms. Each eligible employee must complete an enrollment form.
• First month's premium.
• Broker of record letter. A broker of record letter must be received by Washington Dental Service if commission is to be paid.
